If you're noticing an issue with your embedded Contact Form not showing all the fields since the launch of VSCO Workspace, the quickest way to resolve this is to grab the new embed code from here in Settings › Contact Forms and update that on your website.
In Workspace, head to Settings › Contact Forms and use the 'Embed Form' option here:
Copy the code in the box:
Swap out the current code on your website for this new embed code you've just copied.
IMPORTANT: You'll need to have access to the HTML source code of the page you're embedding this on. Look for something along the lines of 'Edit HTML' or 'Insert HTML'.
